I had the same problem with WP Fastest Cache, disabling JS combine (footer) solved my issue.

@prenschler: I think permission 777 for upload folder isn’t safe.

Worked for me too!

I was using the UploadPlus plugin, which caused the problem.
I found the solution here:

I just removed the db.php file (database cache) from the plugins folder, and that solved the problem.

I am facing the same problem.
Have no idea why.
