was able to fix it by changing the events display setting “events template” to “Default Events Template” (had been set to “Default Page Template”)

When I try to add it, the Install Now option is replaced by a notice that it isn’t allowed on the server (see linked screenshot)

So yes this is resolved by changing the blc_ tables to MyIsam, but shouldn’t they have been created as such anyways? Or maybe an internal check in the plugin that if they somehow get changed to change them back?

but

• value: s:24:”amp-on-off-for-all-pages”;s:1:”1″

Halfway fixed.
Now if I manually set a page to “hide” then it does not include the amphtml – great.

But if I have a page that I have not selected either hide or show (ampforwp-amp-on-off = default or blank) then even though I have the settings set to off for pages (s:24:”amp-on-off-for-all-pages”;s:1:”0″), it still displays amphtml.

It’s as if “default” isn’t catching that the settings are set to off.
