I checked the form here and it seems to be working correctly now and I am not able to see any JS errors in the console. If you have WP Rocket active on your site, it is definitely a good idea to white list the pages where the forms are present from being minified.
As for your copy of WPForms, please know that we are not affiliated with any third-party site that sells the paid version of WPForms.

Are you still getting the warning message when you’re logged in because when I checked the page and inspected the code console, I don’t see any Javascript error that should trigger the warning message that you’ve mentioned?

My apologies for the wait.
I checked your site but you might have removed the reCaptcha in the form or you might have fixed the issue because when I tried to send a test entry, it went through without any issue. Here a screencast for reference.

This issue of duplicate entry is likely observed when the user refreshes the page after form confirmation is displayed. To prevent duplicate entry due to users refreshing the page when a confirmation message is shown, you can consider a couple of options:
1. Use AJAX form submission: When this setting is enabled, the form will not be resubmitted when users refresh the page after form confirmation is displayed. To enable this, go to the?Form builder > Settings > General to select Enable AJAX form submission.
2. Set form confirmation to redirect users to another page:?The user can be redirected to a ‘Thank You’ page instead of being shown the confirmation message. To enable this, go to the?Form builder > Settings > Confirmation and in the Confirmation Type dropdown, select Show Page or Go to URL (Redirect)

Thanks for writing in and we’re sorry to hear about the trouble you’re facing.
I checked your site and it looks like you have a caching plugin installed and activated on the site. We’ve seen cases where the minify and combine script feature of caching plugins causes WPForms script to break which results in issues like you’re getting right now.
When you get the chance can you please try the following?
- Exclude the page or post?where the forms are?from JS optimization by unselecting the Minify/combine JS option in the page or post editor. Please check here for more details.
- In WP Rocket, disable the?Combine JavaScript?files?option then save the settings. For more details, please check here.
Kindly make sure to clear your site’s cache after doing any of the changes above.

It seems that you are using the paid version of WPForms. If you have an active license subscription with us, could you submit a support ticket through the WPForms account dashboard when you have a chance?
