I thought initially it was a server settings problem but I can see a hack is the likely cause.
You kindly outlined precautions to prevent getting the problem. Have you any advice for those that have the problem which either disables their WordPress site or, as in my case, disables certain functions within the WordPress editor (TintMCE Advanced)?
