We are also having this exact same issue after updating a client’s plugins. Our instructions to them included creating the gallery from “Add Gallery / Images” but now the galleries they created that way are missing the fields entirely and need to be manually fixed.

Hopefully the ability to create galleries with the fields enabled at the same time as uploading images can be restored!

I was able to duplicate this on my super minimal setup: Twenty Fourteen theme and only Akismet and Contact Form 7 for plugins.

What I didn’t actually type out in my example is that the dash in “Other – please specify” was actually an mdash HTML entity. The value from the free text field was not sent when the mdash was in there. When I reformatted the value to “Other, please specify”, then the field worked as expected.

Unfortunately it’s not a public site.

I’ll try installing the plugin and testing it out on a site with fewer plugins if that might be an issue. We are using the Dante theme with a decent number of plugins: Breadcrumb NavXT, Contact Form 7, Contact Form DB (I actually didn’t realize this was a separate plugin storing the values in the database), Duplicate Post, Google Analyticator, LayerSlider WP, Listo, NextGEN Facebook, oAuth Twitter Feed for Developers, Really Simple CAPTCHA, Revolution Slider, Simple Redirect, Simple Side Tab, Simple Vimeo Shortcode, and WordPress Importer.
