Looks like this was related to AdBlock Plus. I disabled that and now it seems to be OK. Keeping this in case other people have a similar issue. Not sure why AdBlock Plus thinks these images are ads.

If you use WordFence try deactivating it and then backup then reactivate it. Works for me.

OK, I see that this shortcode may work [wp_email_capture_form] but I don’t know how that would let me choose a specific list. It’s OK for now since I only have one, but it would be good to know about this. Also, the shortcode is not listed in the PDF documentation at all, so I would suggest adding it there. Thanks!
