Could it possibly be that your plugin, the caching plugin and possibly the other plugin that has errors are all conflicting with each other?
Do you know where I can find a tutorial/information on how to create the text widget shortcode instead of the SF widget? On our contact page, I am using a text block and using this code “[salesforce form=”2″]” in there. Is this the same thing?

Hi, if this helps, just to let you know when you click on a webinar you want to sign up for, that is when the error pops up. But you are still allowed to register it seems.

HI, We are going to have a contest and at the end of the trade show we will pick the winner randomly. So we would like to catch all the entrants information. By doing this, we are going to hand out about 500 unique flyers to people with a unique number on it. Once a number is entered, it will not be able to be submitted again. Does the plugin notify you that the number has already been entered?
