Nick, We experienced same issue. Not sure what the trigger event was –only discovered after I noticed a plugin message stating org ID not set. We’ve been using the plugin for years without issue – great solution but could use details on the correct files and actions required to restore.

We’ve added a confirmation or Thank You page that opens upon form submit (native functionality of Brilliant) and designate that specific page so Google Analytics tracks conversions.

Thanks for the clarificaton Nick. Posting update here for others who might have similar issues.

I tried the format as suggested (Name1 | Value1 | Name2 | Value2) and found it populates the pick with both values and labels. Changed format to (Select One: | name1:value1 | name2:value2) and that resolved.
