I put the posted log file entries into a code block as shown below.
The last entry has the 404 response.

The log entries show “200” until the last entry which shows “404” That “404” log entry is generated when I exit from the customize screen. I will be away from the Internet until Sunday afternoon (GMT – 4).

Lastly, I am available to continue this conversation. I will retain the VMware snapshot with the WPForms test configuration for one week, but the contact-me page will not be publicly available unless requested because I need to create content for other pages.

I agree that AMP by design does not function with the java scripts.
As I mentioned above, the WPForms advertised capabilities are not entirely accurate.
As we know, other people have also been misled about WPForms and AMP.
It is not your fault. I am not blaming you.
The advertising should clearly state that the forms from WPForms do not natively function within AMP pages. If an AMP page is detected, the following will be seen, you need to click on this message: “Go to the full page to view and submit the form”.

By default, the AMP plugin appends
/?amp=1

We know ReCAPTCHA v3 will function with AMP as per the following setting from the Google reCAPTCHA v3 configuration: Then scroll down to the bottom of the settings page and select the checkbox next to?Allow this key to work with AMP pages.

From the instructions, Using reCAPTCHA with WPForms and AMP, the following is technically incorrect.
If you wish to display the reCAPTCHA badge on your AMP pages, you’ll need to enable this in your reCAPTCHA settings.

The following statements confirm WPForms does not properly function within an AMP page.
To use WPForms with AMP, you need to click on this message: “Go to the full page to view and submit the form.”
However, in one of your screenshots, you appended
/?amp=1
to the URL, but the page is showing the form. With this, the alerts related to JavaScript files in the Console are expected, since AMP pages dequeue JavaScript files on the page.AMP Plugin
