Same problem here on all my sites. Sucuri is not installed on all of them.

Krumch — Good thought. But I checked that. The URL of the “Back to ___” link just goes to the homepage with no URL arguments of any kind. It looks like the issue has something to do with changing a password for an already logged-in user. (Typically the lost-password dialog is used for non-logged-in users). At least that’s my best guess.

Typo: First sentence should read: “1) I created a link which allows already logged-in users to change their passwords.”
