Can you try clearing your browser cache? The line in your screenshot causing the error has been removed in version 4.06.01. Looks like maybe a cached version of that file is being loaded on your site.

This can be done, but requires a somewhat complex setup, as well as the Formidable API and Form Action Automation add-ons for automated deletion, and the Table View to CSV add-on for automated export.
These are all included in the Elite license.

This can be done with the Pro version, but not with the Lite version.
