Thank you, bnovotny, it is now working. Another plug-in of mine interfered and added those additional FirstName and LastName fields at the bottom. Your plugin is perfect for my needs, after all.

The “First Name” and “Last Name” are at the bottom.

If I add “First Name” and “Last Name” on the “Add Additional Fields…”, they come up twice; these, and the ones on the first paragraph.

How can i move them up? Or how can I remove these and use entries from “Add Additional Fields…” instead?

Just don’t include the last “</div>” and the layout won’t break. It did for me.

Click on Manage Gallery, then click on your gallery Title. The next page will show Scan.
