I believe I did this correctly. In the Network tab, I sorted out the “Status” column and found there were two 400 requests. I copied/pasted to pastebin the request headers, response headers and response.

I contacted my hosting and they said it has to be done through htaccess. I googled around and saw how to update my htaccess and I added the line Header set Access-Control-Allow-Origin “*”

I found the problem.

The single quote on (‘crellySlider’) and (‘your_slider_alias’) has to be written with the single quote next to the return key:

(‘crellySlider’) and (‘your_slider_alias’)

I would deactivate all your plugins except for VFB and then see if it works. Optionally, you could just get a new form plugin

Just wanted people to know that I think I found the solution to this in case anyone was wondering.

1) To get notified if someone submitted a form, it’s under the “settings” tab in your salesforce plugin, then scroll down to Email settings. Fill out those fields with your email addresses.

*A little confusing “Email from name:” and “Email from address:” should have a description of what we are supposed to type in the fields. “Email from name:” sounds like we should type in someone’s name such as Jon Doe. It should have a description to what the purpose is and that we should fill in an Email address.

2) I think to get the extra field to show up in the Lead section in sales force, there is an ID specifically required by salesforce. I haven’t found out what that is yet but that’s the reason it’s not showing up. If anyone knows how to remedy this, please post.

In other words, the solution is put a random number other than 0 or 1 into the value field and the check box will show unchecked by default.
