Well the issue was not with the plugin at all, the issue was that I had a div in my footer that used the same id as a div in the form so they were conflicting with each other, I simply renamed the div in my footer and boom, it sorted it out ??
perhaps someone would know how to make the telephone number so it is ‘not required’ in the form validation? or perhaps remove the telephone field altogether?
