Hi Michael, sorry I did not see this before posting.
* WooCommerce Checkout Manager 4.2.3 * WooCommerce 3.5.0 * WordPress 4.9.8 * Themes tested – Divi, 2017 * No errors
I ended up moving to another plugin but am happy to continue troubleshooting this issue if you want to.
