I see another problem, but I think it is the same problem.

My page with the short code
[link-library-addlink settings=1]
Has the same problem.
It does not show the form fields. But gives the link list too.
And with version 5.8.4.3 I do not have this problem.

I did both reset buttons. I do not know the text on buttons is in English, but one called something like this :
Reset this library
And other button
this library for table layout.

I did get a table layout. And reset that with other button.

Both buttons did not give a solution. I still have this problem.

I did the update automated.
I just renamed the folder to old.
And downloaded the folder from the other website to the plugin folder.

I did change again with renaming folders, back and forward, to new and old, to look at it again.

If there was something wrong with my settings, than renaming folders would not solve the problem I think.

I deinstalled this plugin.
Maybe when I have more time I will look at it again.

I went further to work with other plugin.

I cannot find this in your knowledge base under shortcodes or shorcode

Under shortcode I find a page. But this shortcode is not on the list.

I did find it now, after days and hours searching.

And then it is on a video. Why not in text too?

Advice, please put it in your knowledge bas under shortcode.

That is the same answer you gave with the multi-select input problem. And then the problem Was your theme.

With other themes it works well.

Furthermore when I install your theme Shipyard it works well.

To solve the multi-select input problem :

Advice from the other plugin owner
Add this to style.css,
.search-field input { height: auto !important; }

That was the solution. Everything looks fine now
