That could well be the case – I recently had another issue caused by caching, so I have installed the SG Optimizer from my host and excluded the cart, checkout and account pages so that they don’t get affected. Perhaps I need to exclude all /wp-admin pages, or is there a better way to target this problem?

Thanks for your help with this issue and sorry it has taken me a while to get back to you.
Just to recap: when doing refunds and leaving the box Restock refunded items unchecked, the item is still being added back in stock.
I have finally had a chance to check this on a staging site, and it works fine, I can’t replicate the issue there (with all plugins active – so this doesn’t seem to be caused by a plugin conflict).
So, I wonder if this could be a server side issue? A few months ago I changed something to do with CRON jobs, this isn’t something I really understand, I followed the instructions they gave me: https://www.siteground.com/tutorials/wordpress/real-cron-job/
Could this be connected to the issue I am describing with refunds? And were these changes they advise a bad idea?

I just realised that the ‘Email Invoice’ is coming from your Print Invoice and Packing Slips extension, I have emailed them.
Your ‘Email invoice / order details to customer’ is indeed creating an order note.
Not sure what the difference is between these two options!

Thanks for your message. It is still happening and all of my plugins are up to date. I have a staging site but haven’t had a chance to start testing different scenarios, such as if it happens when refunding from Processing or Completed status, etc.
Anything I might want to look for or try when troubleshooting would be helpful!

I have also found that leaving this box unchecked doesn’t work – it is adding the items back in stock regardless.

Thanks for the quick reply. I will look continue to collect more orders before I can get an idea of a cause from that.
All plugins are up to date and they’re all popular ‘big name’ plugins that I hope have sorted out any conflicts between each other, but who knows.
The caching issue raises a flag, I am with Siteground and they do have caching, and I do occasionally ‘flush’ it. I’ll contact them to ask if they can remove it from those pages.

Thanks for the quick reply and answer. Is there a way to enable the shipping methods/zones on the Admin side, albeit with an option to override them if necessary, as I described above? I appreciate the flexibility Woocommerce is trying to offer on the backend, but in most cases I just want to quickly and simply add our Standard shipping to the order, especially as it is usually a phone order that requires this manual entering.

I’m not sure I understand. We have genuine orders coming through on our site, between 15-50 a day, but on the analytics it is showing 2-5 transactions a day.
So there is plenty of data there – what would be different about my test purchase?

I have exported the orders and removed the orders I dont want to import again from the spreadsheet. I also deleted the columns for order no and order id, however the import hasnt worked.
Should I leave those columns in but delete the values inside? Or what do you mean by unmapping them exactly?
