To add to this issue, I am unable to use this plugin on my site that is ran over HTTPS connection.

Okay, I added these and it worked:

I have tried all of the below:
https://www.coachburt.com/ https://www.coachburt.com www.coachburt.com/ www.coachburt.com coachburt.com/ coachburt.com /
What else can I do?
