It was good before, not sure what changed. I just fixed it by readding each field and deleting the existing one and seemed to fix it. Thanks!!

When I make edits to just a simple text on the form it updates the website instantly. But all the radio buttons and checkboxes are gone.

That is the correct photo you sent. Apparently it just takes some time to update? It had been an hour or 2, but on mobile and desktop it appears to have corrected itself. I checked my host and SSL is enabled. Thank you for looking into anyways, much appreciated.

Keep up the good work, this is such a great form builder. I just needed a basic waiver for a sports facility and everybody wants to charge $20 a month for a signature. Thank you!!
