Just for some added information, it is not a plugin. I deactivated every single one and still had the problem after refreshing.

As far as discussion settings, they are pretty typical:

Allow comments on new posts
Comment author must fill out name and e-mail
Users must be registered and logged in to comment
Enable threaded (nested) comments 5 levels deep
Email me whenever:
Anyone posts a comment
A comment is held for moderation
Don’t show avatars
Default avatar Mystery Man

I’m not sure where I would see the html in the httaccess file. Looking.

Steve, how did you access the file permissions? I’m having the same problem!
