I stripped those out and it was still using the tag line. So in the Titles section of the configuration I used %%excerpt%% in the Meta description template and now it’s working. Thanks.

Make sure your API Key is the correct one, do not use the unique ID number… (at least that’s what I did at first)

I have the same problem, I tried deactivating all but your plugin and it still only shows Form 1. I am currently using wordpress 2.9.2
