Okay, I found the Mailer Debug tab.
In the bottom portion of the report (below the body of the email), it shows valid To: and Bcc: entries, but at the top of the report, only the Bcc: entry is listed, and that address does not get any mail. The submitter’s email address does get an auto-reply.

Now, maybe I misunderstand how this works. If you mean that you can put a chat for one Facebook page on up to three sites, then I guess you are honoring your “up to three websites” statement. But that’s not what I need. I need to put a Facebook chat to two different Facebook pages on their respective websites (so not even three). But apparently I cannot do that, and $10/mo ($120/yr) is not a price I can pay for such a simple piece of functionality.
So let me be clear on this. You’re asking me to post images of my OAuth client id and secret codes in a public forum?

As the uri was copied directly from the result Google gave me (about 3 attempts, including a complete do-over and being careful not to accidentally get leading/trailing spaces,) I’d have to say there is little doubt it was correct.
Apologizes. I appear to have answered another person’s thread on the exact same issue. So apparently this is more than just an isolated issue, but I responded out of line with the original poster. I will re-post to my own issue thread.

WebDevStudios replied that they were unable to reproduce the issue and suggested I look at “the settings for your own post types” for a possible clue.
When reactivated to see what settings they could even be talking about, the JetPack Feedback item remained in place. I have no idea what was causing your item to fail to register on the menu, or why it succeeded in the absence of Custom Post Type UI, but I think the long and short of this is that I just chased my tail in a full circle and nothing has really changed.
In looking over the Feedback entries in the db, what I can tell you that’s different about the “inside out” submissions I’ve received is that they are not there at all. In other words, JetPack emails me the submission, but does not record it in the database.

It’s the “Custom Post Type UI” plugin that has broken your dashboard entry.
Looking at another one of my WordPress websites that has the JetPack Contact Form but no Custom Post Type UI plugin installed, I noticed the Feedback item was present. Looking at the HTTP GET parameter in the URL generated by Feedback, I was able to determine you are using a custom post type. That gave me the idea to disable the CPTUI plugin on Girls Rock Radio, and -viola- the JetPack Feedback item appeared.
I have reported the situation to WebDevStudios, the author of Custom Post Type UI.
I still need to look over the JetPack Contact Form entries in the db that were received inside out vs normal to see if there’s something different between them, but we’re getting closer…
Deactivating and reactivating the module has no effect – Feedback dashboard item remains missing.
