I have checked accordingly on our local server and it works properly. The added custom field values were sent along with the email successfully.
Based on your given information, please confirm that the ‘meta_key’ email tag {ARM_USERMETA_meta_key} you are replacing is the same as the Meta Key of the field (not field label). 
You can get the ‘Meta Key’ for the custom field from ARMember -> Manage Forms -> Edit Signup form page click on the field’s setting option. That’s it.

So, for example, if the meta key is ‘file_dsd23’ then, the email tag you need to add as given below:
{ARM_USERMETA_file_dsd23}

Kindly check accordingly and let me know how it goes.

If an appointment is not created when payment is done with PayPal then kindly double-check IPN Settings. Go to PayPal Account -> My account –> profile –> my setting tools –> instant payment notifications –> update –> choose IPN settings.

Still, if the appointment is not created, BookingPress provides the payment debug Logs from the BookingPress -> settings page -> Debug Log tab -> ‘Payment Debug Logs’ section please enable the PayPal method Logs. and you can check by booking an appointment.
