    OK they switched servers on me several months ago without telling me. Contacted support today, and they said my user in the mysql database was not pointed to the correct db. Asked them why it was OK before, and they didn’t know. However they fixed it, I got into wp-admin and updated everything. Now it is working like a charm!

    Thanks, that does the trick!

    I contacted my host, and this is what they found out:

    On checking the error logs I could see that the issue was due to the mod_security rules added on the server and you can verify it from the logs apsted below,

    ———————
    [Tue Apr 15 19:55:42 2014] [error] [client 67.4.198.145] ModSecurity: Access denied with code 406 (phase 2). File “/tmp/20140415-195541-U03G-cc6uisAAGSqfo4AAADC-file-m3xAOQ” rejected by the approver script “/usr/sbin/clamscn.pl”: 0 Unable to parse clamscan output [ERROR: Can’t open file or directory] [file “/usr/local/apache/conf/modsec-maldet.conf”] [line “17”] [id “99999”] [hostname “nissidesign.com”] [uri “/new/wp-admin/media-new.php”] [unique_id “U03G-cc6uisAAGSqfo4AAADC”]

    We have now whitelisted the domain from mod_security rules and this will resolve the wordpress issue.

    This DID solve the problem.

    It looks like something is going on with media_new.php, so I downloaded it from the www.remarpro.com site, and uploaded it to my site that I am working on, and it did not work. On the media page, however, before it gave me the http error. Now that is gone away and replaced with the 404 error as I stated before.

    Downloaded the .htaccess file, and it says this:

    # BEGIN WordPress
    <IfModule mod_rewrite.c>
    RewriteEngine On
    RewriteBase /new/
    RewriteRule ^index\.php$ – [L]
    RewriteCond %{REQUEST_FILENAME} !-f
    RewriteCond %{REQUEST_FILENAME} !-d
    RewriteRule . /new/index.php [L]
    </IfModule>

    # END WordPress

    This is also happening through the browser upload. That is when I get the 404 error. However when I go to Media, and try to just upload a new image, it gives me an Http error. So neither one works.

    I have checked all of the file permissions, and the directory folders are all 755, and the files are all 644 except the wp-config file, which is 600. I deactivated all plugins except for Akismet. I am still unable to upload images.

    I did contact the host, and they said that my IP is not blocked, and they tried uploading images through the file manager and it works fine. And this is a 404 error, not a 406 error (which I have never seen before).

    And the themes.php file IS in the admin folder. I tried in IE as well, and still the same. Also, when I try to add images in media mananger, I get http error. I also re-installed the new version of WP, and nothing. Do I have to go back to an old version???

    Same here. This is really irritating, and either it gives me the 404 error theme.php is not found, or it says my IP is blocked. Checked with the host, and it is not blocked. It is something in the current revision of WordPress, and I hope they get it fixed soon.

    I am having the same problem. First it said my IP was blocked, and then it gave me a 404 page not found for theme.php. I checked with my host, and they said nothing was blocked on their end, so it must be the current version of WordPress. Have not been able to load any images since the current revision.
