So, yes, please do look into it whenever possible. If you Google “By submitting this form, you are granting: , permission to email you.”, you’ll find a lot of results showing sites with the same issue, so it looks like it could be a bug.
I could manually fix it in the class-display.php file, but I’d rather not, as I’d probably have to re-fix it with every update…

Thanks Michael for the prompt response. However, all the “Account Info” in our account is correctly filled out (including our company name) and we still have the blank results…
Any other ideas?

Yes, that did it! Maybe you could add that clarification under the “File Name” field to prevent confusion.

Same issue here!
Just installed the plugin, everything works great except for the actual download. It downloads the file WITHOUT the PDF extension (if I add it manually after downloading, it works well; but most of my users won’t figure this out).
Suggestions please!!!
