I saw the same hack a month ago. The injection was in wp-option table. There were no any other files or tables affected in my case.

You can try to disable your plugins and themes then upgrade your wordpress and enabled them
Permissions for the files should be 644. folders – 755 As I see you have 404 error. It isn’t permissions problem

I think the problem with permissions of your files/folders. You should set 755 for all your folder and 644 for files.
or not correct rule in .htaccess file.
