Our first step was to inactivate all plugs and switch theme to default 2015 (see first post). The problem was still there.

Another update.

By installing Advanced Tony MCE plugin, we have an upload media plugin option.

But now the media library doesn’t load and if we try and upload a file, we get an error: ‘you do not have permissions to load files” We have also got a different error when trying to upload: “An error occurred in the upload”

Have checked the permissions on upload folder and sub folders which are 755

Forget to mention, we upgraded from WP 4.0 to 4.1 in the hopes it might clear the problem

I have same issue on wordpress 3.8.5, but I was starting from scratch adding a gallery to a new page and the nextgen icon never appeared.

I have similar problem. My next and previous links work in month view and single event view but not on list view.
