That may be part of the problem. I’ve found that the visits graph becomes more useful when you have a great deal of traffic. Otherwise, the data you receive will not be statistically significant.

It doesn’t use a save button because it uses ajax to automatically save the url once you select it. If it doesn’t save then you may have javascript disabled or a conflict with another plugin.
Once you select the country URL, you should see a success message saying the URL was changed.

This could be because you are on a shared ip or that you have too many keywords being reported. I suggest you use AuthorityLabs to get your rankings (10 keywords free). You can also try SEOBook’s Rank Checker but you have to do that one manually.
I would be happy to see what you’ve corrected. If you don’t mind, send the corrected files to david at kwista dot com.
I’m not sure if the Rank Reporter could have caused the problem. It could be any number of plugins or other issues. Unfortunately, there is not a way to complete the captcha automatically.

The error appears when you try to add a keyword or click the “Check Rankings Now” button?

This is probably due to the fact that Google is blocking your IP from reaching google.com. This is usually caused by others on your same IP who have spammed or overly pinged google.com.
I may add the ability to add another IP to the reporter. But for now, the plugin probably won’t check rankings until you get your own IP or switch hosts. Sorry about that. I don’t think there’s anything I can do.
There are other options to check rankings. AuthorityLabs will do 10 keywords for free. SEOMoz will let you track 5 rankings per day.
I believe this is a problem with your host. You have to make sure CURL is enabled and that Followlocation is allowed.
