Actually to correct my earlier statement, it isn’t every page in the Admin backend. Some pages where it does show: /wp-admin/options-general.php and /wp-admin/widgets.php.

It maybe has something to do with file system access or specific options – on the General settings page for example, the error is shown right next to the “site icon” field.

I’ve given it a quick test on one of my local installs, worked fine for me. I set the custom header on a few posts, and also tried removing it, all looked good.
