I am having the same problem after upgrading from 2.7.1 to 2.8, using Windows XP. When uploading, after the “crunching” instead of showing a thumbnail of the image I get that “broken image thingy” thumbnail and then the same problem you see on the “Add an image” dialog.
Please someone enlighten us as to a solution.
