Ok. We are testing and patching the product, but have not been able to consistently reproduce this issue.
Do you have any permalink based or other redirection plugins installed?
Name of the store is not available. All of the other items are in the Pro Pack as is CSS themes for the plugin.
Check the main settings page (general settings) and map settings page.
The way that geocoding works has NOT changed in recent release.
Google, however, has clamped down on their API calls and is now much more stringent on calls that go over limit. If you are having limit issues then you likely need to add your credit card info on the Google API system so you can make more requests, ESPECIALLY if you are using the “center map on users location” as that requires an API call every time someone visit the locations page.
@trouble – we respond to forum posts about once/week. If you need a faster response we do have paid support options available.
The best way to modify the layout is to use the CSS files and create a new theme that moves/displays/hides those items.
Changing code will break any time a new release comes out, which is about once/month.
It is available in the current 3.1.5 release.

No date has been set at this time.
You can edit any of the pages manually after they are created.
We have a new version of Store Pages with templates on the way which will give you a lot more control over the initial layout when pages are created. We expect to release that in the next 30-45 days.
Make sure you have Pro Pack installed. We are looking into Store Pages and running testing in preparation for a patch release coming early next week.
That needs to be done with permalink management. There are a number of plugins that will handle that for you.
The best way to discover CSS dependencies is to use a developer tool in your browser such as Firebug for Firefox.
