You can change the Subject Line of your emails by editing your form, navigating to the “Emails and Actions” tab, and clicking on your Email Action.
In the settings that appear, you can change the Subject.

If you would, please edit your field and expand the “Advanced” section. Here, you can change the “Label Position” setting:

From your screenshot, it looks like there could be some incomplete Conditional Logic within your form.
If you would, please navigate within your Ninja Form to Advanced > Conditional Logic, and take a look at any logic lines that say “Select One”.

I’m sorry to hear that you are experiencing this. On Ninja Forms v3.4.17, I added a new field, clicked the star to save this as a Saved Field, but this worked on my website.

If the custom code you are using can update the hidden field (such as by using jQuery), then you could map that hidden field within the “TO” address.

https://developer.ninjaforms.com/codex/changing-field-values/Alternatively, if your pages contained a querystring, you could have your Ninja Form pull that value out of the URL. Within your form, you would then add an Email Action for each page/email group and set the conditional logic to only process an email based on what the value of the hidden field is.
