If you don’t deselect the CDN “Enable” box, it will ask for the hostname info.

I was having permissions problems like this and finally enabled SuExec for Apache — everything works great now.

I don’t see a restricted mode. I’m using WPT Pro. Am I missing it?

I just added a page counter in the code. With the SEO Ultimate, (all cache disabled) it counted 142 on the single-post pages, and with the SEO Ultimate disabled, the query count went to 84.
