Which language have you activated?
I had the same problem, but in my case there where also the error:

TypeError: Language code must have format 2-3 letters and. optionally, hyphen, underscore followed by 2 more letters

In my case, the language “de_DE_formal” (Deutsch (Sie)) was activated.
After I set the language to the “de_DE” (Deutsch) the error was gone.

When I change the language from “de_DE_formal” to “de_DE” the javascript errors are gone.
I forgot to post one javascript error:

TypeError: Language code must have format 2-3 letters and. optionally, hyphen, underscore followed by 2 more letters

This error seems to be the bad one.
So it seems there is a problem with the formular german translation.

I tried to duplicate the issue on my local pc. And as you said, there it worked fine.
Then I figured out, that the issue only occures, when I activate the plugin for the whole network.
But when I activate the plugin for each part of my multisite step by step, It seems to work like a charme.

So, as I said, only when I activate the plugin for the whole network in the network-managemend, the issue occours
