You shouldn’t be experiencing that issue with the latest version of the plugin. Anyway the solution changing the load method in the troubleshoot area is also valid.

Do you mean authentication for accessing the public booking form. A solution may be to publish the booking form into a page protected by some login/membership plugin.

Hi @lizmm,
The plugin isn’t enqueuing the form scripts in the page. There may be a problem with the theme or with Elementor and the way the booking form was published. Please try publishing it with the booking form shortcode to check the result in that case.
Another option is to change the “script load method” to “Direct” in the troubleshoot area of the plugin (located under the Appointment Hour Booking menu >> General Settings >> Troubleshoot) as mentioned by @adnoart .

The 503 error on that page seems to be related to some restriction in the server side. Are you using the same hosting service for all the websites affected? If yes, please ask them for the detailed error log for that error to know what restriction is causing the problem.

Please check if those appointments automatically marked as approved have a time-slot selected. Note that the status is linked to the time-slot selected in the calendar field.
Mark the calendar field as required to be sure that all the bookings proceed with a selected time-slot:

I recommend you to check the requirements for sending emails from your hosting services. Check also if you need to set up SPF or DKIM email authentication for your domain (required by Gmail and other services for accepting the emails).
An email log plugin may help to know on which side (plugin, WordPress, server) the email is being lost.
