You have to delete the plug-in in your wp-contents/plugin folder.
I had the same problem.
This plugin does not work with my WP version : 3.8

Same problem with these CHMOD changes.
I’ve tried a copy of this website on my local server, and captcha appears perfectly.
So I have a problem with my server configuration.
I see in GD support that I have these lines in local, and not on the other server.
XPM Support enabled

XBM Support enabled

-> this URL show me a background captcha code, but no letter an no number
My server has GD support enabled, GD version 2.0

I have no PHP error displayed. and no JS error.
