As far I know it is impossible to change appearance of checkboxes and radio buttons. The best way to achieve that is to hide checkbox and show div which will look like checkbox. There is many guides which will show you how to do this only using css.

We will add this functions in future releases.
There was, indeed, hidden textarea under the checkbox. It was needed in older versions of Contact Form 7. In next release it will be removed.
When it comes to visibility of the checkbox in Contact Form 7 only for logged users, we can’t induce such an issue. On WordPress with default theme, Freshmail plugin and Contact Form 7 everything runs OK. Could you provide more information?

You need to set horizontal padding in form styling options and when it comes to this huge gaps between inputs, your theme adds to every input additional margin: https://scr.hu/2w04/tbhrl
Please change your theme styles for inputs and everything will as it should be.
