In reply to: General Settings Error: options.php "Not Found"The .htaccess in / looks like the standard one, and judging by the timestamp, I did overwrite it earlier today when uploading fresh files. There is no .htaccess in wp-admin/.

I should have mentioned I tried switching back to original theme with no change. Below are the results of the steps and tests you suggest above.
Plugins:
1. I disabled the plugins using the FTP method. No change.wp-admin & wp-includes:
2. I deleted and re-uploaded both wp-admin/ and wp-includes/ and the root level files, except .htaccess and wp-config.php (from a fresh download of Wp). No change.Is there something is in the database that would control which file is the form action?
