Well, what a wonderful day was spent troubleshooting this. I wish I could say I found the issue – but it was ours, somehow, in our custom post type “Reports” – not in any other custom post type. With all plugins disabled. With functions.php turned off by using the 2015 theme. It worked fine in a standard install on another domain. Crazy!
However, I did solve it by turning off the notification email in the discussion settings. It seems your plugin still sends the notifications if this is turned off, thank goodness. It removed the duplicate!?
In the process of troubleshooting this, I removed our Mandrill SMTP, added the WP Mail SMTP plugin and setup our Sendgrid API.
We are using a custom post type “Reports” in your plugin. However, both the plugins notification and the original WP notification are both sending to the post author when a comment notification is sent.
More troubleshooting soon.
Thanks for your response – but no, I’m just using Mandrill’s plugin, and sending them to their transactional template. However, I run a very custom membership community site (buddypress) that uses 89 plugins to operate and 1100 lines of functions.php
Luckily I work with a development server too!
I am in the midst of switching everything to Sendgrid, I’ll do a test today to see if that may be where the problem is, and report back. It could be an easy fix if so.
However – it sounds like @trylobyte here is having the same issues without Mandrill?

Still an issue with 3.47.11 and 3.0.88 premium.
I did a quick fix (albeit manual and more work) by replacing the unsubscribe link with a quick `mailto: [email protected]?subject=Unsubscribe’ until this is resolved.

None of the links gave me 404 errors at all prior to your plugin installation, but did once your plugin was installed. Actually, it brought down most of my buddypress based community until I had a chance to turn off your plugin. My inbox was full this morning of people unable to navigate.
If the buddypress pages are not giving 404’s before your plugin is installed, how can this be buddypress’s fault? Isn’t instead your buddypress revision you recommend so that your plugin can read things and work correctly in a buddypress environment?

I didn’t have any 404 errors before the install of your program though, so if it’s a bug – it’s a hidden bug that only came out when trying your plugin. Perhaps you should state on your main description it does not work with Buddypress.

I would rather pay for a plugin that stays current than a free one with a slow upgrade cycle. It’s good for people to know this authors approach and response, and hence I reviewed. But after this diatribe, it would have been easier to have said nothing, and not helped other mission critical sites. Naw. I’d do it again. ??

I have kept software going for communities thru multiple CMS over decades. They evolved or they died. I of course use dev sites to not cause me upgrade headaches. This broke. The dev didn’t want to fix it. I showed alternatives. It’s so simple, why are you reading so much into a review of a dying plugin? Is this a counseling session?
I’m having a similar issue on 2 of my sites. Yes, i changed the database prefix, but the site works fine without the plugin installed. If I try to reinstall the plugin, the site locks up and goes white. If I delete the plugin manually, I can restore access to the website.
Interestingly, I am not able to install it on another domain that is in the same user account as the one that is not working. It has the same symptoms, yet was never installed prior. I hope this gives a clue.
I like your plugin, and use it on many of my other sites.
The site where it is NOT working is my most hacked domain, and I would really like to get this running. I am happy giving a donation for your attention. Thank you.Forum: Plugins

I had another site hacked that had used the w3 total cache plugin. Looks like there’s a vulnerability.
