I am having the same problem but I can’t find how to add a fieldset to my form.
Wordpress Version: 3.4.1
Plugin Version: 0.91 (recently upgraded)
If it is supposed to be on the FIELD TYPE select is not. These are the options it shows:
captcha field
checkbox
comments area
datestamp
followup page
hidden field
large text area
page break
password box
radio buttons
recipient selection
required checkbox
required password
reset button
selection dropdown
single line text box
small text area
spam blocker
submit button
submit image
system field
unique id
wp user id
Please help!
