As the creator, I have to clarify some points of your review.

1. Regarding the booking form styling.
The booking form is designed to be integrated with WP themes seamlessly. If your theme is fancy, the booking form is fancy too, with the same style. We think this is a very competitive feature comparing to others rather than a defect.

2. Compatibility
We do not use the Chosen library. It is WP Jump Menu using it. We use jQuery Autocomplete for the client search field. WP Jump Menu loads their own jquery.ui.position.js file rather than WP built-in lib which causes the problem. You might contact WP Jump Menu to resolve it.

3. Premium editions
Customizing the booking form is a feature only included in the business edition and above. Our team spends lots of effort and time to add advanced functionalities in the premium edtions. The free version mainly satisfies the simple scenario. As to $22 plugins in the Codecanyon market, we believe serious projects can hardly rely on it.

Providing a specific day is only supported in Business and upper editions.

It could be a theme conflict or a plugin conflict. You can switch to the default WP theme and check if the problem still exists. You can also deactivate all other plugins to see if the plugin works or not.

Could you go to provider page and check if you set up the work schedule?

Viewing all appointments for all providers in calendar view is not supported in the free edition.
