just ask wpengine to add an exclusion in their caching for this plugin and it should work again.

I was having this exact problem, and your solution fixed it. Phew!!

have updated builder and problem solved.

Ok, I deactivated current theme (ithemes builder) and activated twenty twelve theme, and the submenu items appeared instantly!

So have asked ithemes about this issue also, looks like something to do with their theme.
