Hi!

The problem is that that the pop-up form i’snt coded to allow relocated WordPress installations.
Below You will find a “quick-fix” of the problem.
exchange the content of your ./wp-content/plugins/wp-tooltip/js/button-tooltip.php file with the code below and it will work.
