I’ve got the same with one post. I have noticed, that switching permalink settings in default mode resolves the problem, so I am keeping research in this direction. Additionally I am using WordPress SEO by Yost and WP Fastest Cache – both are managing .htaccess, so maybe there is a problem.

Try to disable plugin cache in plugin tools. I had the same problem and it helps me.
