I’ve done some testing with the above fixes and found more issues.
Using the Firefox plugin User Agent Switcher The titles for most pages and posts was set to “this is a yoast test title” if Google bot is selected.
I have tried a few different versions of Yoast SEO but it appears the last 3 are borked somewhat.
I think the /wp-admin and related non user paths are automatically blocked via WordPress which causes this error, would be nice for Webmaster tools to be more specific though.
I doubt Yoast has given up on anything – these things happen.

I noticed a massive traffic drop which hasn’t recovered yet – homepage dropped rankings massively.
Google webmaster tools was no help though displayed the category 404’s so checked with Bing webmaster tools which told me my <title> was completely missing for my homepage.
I used the Strip Category base plugin as suggested which appears to work though I am not convinced the WordPress SEO plugin is rewriting titles correctly in general. Also had to enable “Use meta keywords tag?” as keywords were also not generating correctly for categories and tags.
Using W3 Total Cache here also.

In reply to: no visual editor when writing !Had a similar issue though the reason was my webhost has PHPSuExec installed.
This means you need to create a php.ini file to allow certain functions.
