    Perhaps you can add the “re-save permalinks” tip to the installation notes? That’s all it took to get it to work for me, and adding that tip could save you lots of forum posts.

    My two cents: Why require both the title and description in the question post type?

    If a user has a simple question that fits in the question title, don’t make them also add a description. I suggest making that field optional.
