FWIW, I was having the same problem…and then I deleted Akismet and the problem seems to have gone away. Hopefully this info might help.

Literally, nothing has changed, and all of a sudden it started misbehaving

I did try those things and none have worked. Also, b/c this is a production site, we don’t change things frequently. It appears as if it started happening all of a sudden.
