I facing this problem too. I can’t leave comments, neither logged in nor logged out.
Anyone found the cause of this?
I believe the problem is that AAM, when installed, automatically assigns you the Super Admin role. This is ok on a Multisite, but maybe unnecessary on a WP regular install.
I decided to uninstall the AAM plugin and install the Members plugin. When I revised the permissions using the Members plugin, I noticed that the Admin role actually had more permissions activated than the Super Admin role. I decided to take a closer look, and discovered that all the permissions related to Document Revisions where deactivated on the Super Admin Role (the role that AAM assigned my account – it stuck even after uninstalling it). On the contrary, all the permissions related to Document Revisions on the Admin role were activated. I believe that’s why the Admin role had more permissions than the Super Admn role, and why I can see Document Revisions on any Admin account I create, but can’t see it on the Super Admin account.
Maybe a work-around for this could be to temporarily install the Members plugin to fix the permissions on the Super Admin role. But I cannot confirm this, as I uninstalled AAM completely and decided to stick with the Members plugin.
If someone decides to test this out, please post back so we know if it works.
Issue was due to Advanced Access Manager plugin. I uninstalled it, installed the Members plugin, fixed some permissions related to Document Revisions and everything now works great again.
I’m still having this issue, and some other users as well. Please take the time to check this up.

Did anyone find a solution for this?
I even tried to deactivate AAM and still can’t see the Documents Dashboard menu item. Seems AAM alters the basic configuration as well, or is leaving traces of its security setting even after uninstall.
Maybe if a few of us follow up on the post I made the author would be “motivated” to look a this issue. See my previous post (above) for the link where I originally posted the issue

But now that I take a closer look, I too have the Advanced Access Manager Plugin installed, and also believe that the issue is due to a conflict with that plugin.
Maybe we all that are having the same issue should file a bug report on the Advance Access Manger’s forum. At least I’m going to try that.
There seems to be some kind of imcompatibility with WordPress in Spanish (I mean, a site installed with the WordPress Spanish instaler available at https://es.www.remarpro.com) and WP Document Revisions. I tested it on a site intalled in English, and it runs without problems. But on a site installed with the WordPress Spanish instaler the Menu item on the admin panel is not shown.
I would really love to use the plugin with my site’s team. Could the developer take a look a this?
