Consider this issue RESOLVED…
I had to recreate the FORM from scratch in order for it to work. Thank you for explaining the problem and its solution as you have pointed me in the right direction.

I love the form and its ease of use and functionality. There is so much in the free version, but I’m going to signup for the PRO because of your fast support (Kudos).

Tiered Pricing Table for WooCommerce plugin was causing the issue here, but this plugin was working fine with WCFM Frontend Manager version 6.7.1.
The Tiered Pricing Table caused an error in the woocommerce status log; everything seems to be ok with wcfm after deactivating the Tiered Pricing plugin.
