What do you mean by “refresh” my permalinks?

We have about a dozen administrators and two super-admins.

No…we absolutely were running 3.2.1. I conduct a test update as soon as every new version is released, then update the entire network after making sure there are no plugin or theme problems. Same procedure when 3.2.1 was released.

Yes. I thought about disabling it as a test.

Since the site is not live yet, I could probably just remove it.
